Skilled Nursing

Our agency provides Skilled Nursing in the patients home. In coordination with the patients primary care physician or allowed practitioner, the nurse will assess the patient at time of admission and create a plan of care to promote and managed the patient care and progress. With a combine efforts of the agency’s certified nursing assistants, license vocational nurses and physical therapists and assistants. The agency uses a “team” method or working and coordinating together to best treat and care for the patients of Shasta Family Healthcare Services.

Certified Nursing Assistants (CNA)

Or CNA’s are assigned tasks by the skilled nurse to assist patients with activities of daily. In addition to monitoring patient vitals, the CNA, assist patients with personal grooming and hygiene, light housekeeping, 1 to 2 loads of patient laundry, and work in combine efforts as authorized by the skilled nurse.

Physical Therapy via Telehealth

Shasta Family Healthcare Services provides physical therapy services via telehealth: before physical therapy can start a physical therapy evaluation must be conducted by the physical therapist. The agency’s skilled nurse will order the physical therapy evaluation and schedule the date and time the coordinates with the patient’s preference. On the schedule date the agency’s physical therapist assistant will arrive and connect via ZOOM with the physical therapist. The PTA or physical therapist assistant will follow all instructions and assist the patient in the evaluation.The evaluation report is reviewed by the agency’s skilled nurse and the patients primary care physician/allowed practitioner and the exercises assigned per the evaluation is conducted by the pta.

How to obtain Services

Shasta Family Healthcare Services requires a signed physician referral for all new patient admissions. To obtain our agency’s referral form, click here to download a pdf or contact our office and will fax the referral form directly to your physician.

Costs

Shasta Family Healthcare Services, Inc. currently provides care on a fee-for-service basis. Generally, the patients are covered by their health insurance policies, and/or state/federal entitlement programs.

In circumstances when the patient is not eligible for assistance through these programs, only then are the patients billed for services provided. During the first home visit, payment arrangement will be discussed. Please contact us for more information.
